CBM Membership Questionnaire

Thank you for taking the time to answer the questions below in writing.  By answering these questions, you are helping us in a significant way to most effectively serve you and the congregation.  When completed, please give a copy of your answers to Ryan Lambert. In addition, each prospective member must verbally share their personal testimony of coming to faith in Yeshua with a CBM elder.

1. To become a member at CBM, you must be a believer in Yeshua and you must have publicly expressed your faith by being fully immersed in water through the act of t’vilah (baptism).  Would you please share when, where, and by whom you were immersed?
2. Are you in agreement with the “CBM Membership Statement of Faith”?  If you disagree with any point, would you please communicate this to us.
3. Are you in agreement with the “CBM Doctrinal Statement”?  If you disagree with any point, would you please communicate this to us.
4. Are you willing to commit to follow the “CBM Membership Covenant”?
5. If you develop a conviction over time that conflicts with any of the above documents, will you agree to share that with one of the elders of the congregation?
6. Are you presently a member of any other congregation or church?
7. In what ways do you see yourself as contributing to the growth and life of our congregation?
